Once the form is filled out, please send it via email to info@anautasticdream.com or drop it off at An Autastic Dream's address: 3726 North Main Street, Farmville, NC 27828. Make sure all sections of the application are filled out before submission. Remember to include two valid proofs of identity like a driver's license or identification card (front and back), a non-expired passport, birth certificate, or social security card.
Application Period: Applications are accepted from May 1st to September 30th each year. Applications will close early if funding is exhausted.
